2008-2009 College of Health Sciences Scholarships

ScholarshipsScholarships are a great way to provide additional financial support for eligible students. As a CHS student, you have many scholarship opportunities available. CHS typically awards more than $40,000 per year in scholarships to our own students. That’s in addition to scholarships that are available through UWM’s Financial Aid Office.

Use the Application form to get started (download below). Most scholarships require that you submit information in addition to the application form (e.g. Letters of Recommendation, Transcripts, etc.). Please carefully read all information about the scholarship(s) for which you are applying. For more information, email or call the scholarship contact or your academic advisor.

Important note about downloading the pdf version Application form: Status of scholarships is subject to change, new ones are often added, and there are some available that require a separate application process and are not listed in the brochure which is attached to the Application form. While the application form is current, always check this page for the most comprehensive and up-to-date listing of scholarships available.

The deadline for submission of applications is January 31st, 2008. Scholarship awards will be determined March 31, 2008. All applicants will be notified by mail. Awards will be presented at the annual CHS Awards Ceremony on May 9, 2008. Some scholarships require that you submit information in addition to this application form (e.g. letters of recommendation, transcripts, etc.). Please read all information about the scholarship(s) for which you are applying, and follow the requirements accordingly. All materials must be submitted by the deadline. Only fully completed application packages will be considered by the selection committees. All information provided in this application will be treated confidentially and used only for consideration by the Scholarship Committee. Students who have been found guilty of academic misconduct are not eligible for scholarships. Students must be enrolled in UWM's College of Health Sciences in the 2008 Fall semester to be a scholarship recipient. Availability of some scholarships may be subject to change without notice.

Undergraduate Scholarships

CHS New Freshman Scholarship
$1,000 Award
Eligibility Requirements:Undergraduate admission to CHS and full-time status starting fall 2008, 23 or higher ACT score, minimum 3.0 GPA in high school, and participation in extracurricular activities. Submit official high school transcript with application.

CHS New Transfer Student Scholarship
$1,000 Award
Eligibility Requirements:Undergraduate transfer admission to CHS and full-time status starting fall 2008, minimum 3.0 GPA. Submit official transcript from each college attended with application.

Dean's Advisory Board Scholarship
$1,000 Award
Eligibility Requirements: Undergraduate status, minimum 2.75 UWM GPA, demonstrated financial need and volunteer service. Undergraduates must have a minimum of 30 UWM-graded credits by the end of fall semester 2007, have Junior or Senior standing and be admitted to a CHS major (students with pre-major classifications are not eligible). Student must also be a member of a CHS student organization.

Dean Hanus Scholarship
$1,000 Award
Eligibility Requirements: Undergraduate Senior status in fall of 2008 in the Occupational Studies program, minimum 3.0 GPA and career interest in occupational health and rehabilitation.

Eligibility Requirements:Undergraduate enrollment in Health Care Administration as of fall 2008 with overall 3.0 GPA.

Eligibility Requirements: Full-time minority undergraduate. Selection is based on academic achievement, financial need and/or volunteer service.

$1,000 Award

Eligibility Requirements: Undergraduate enrollment in Medical Technology submajor of Clinical Laboratory Sciences program, U.S. citizen and Southeastern Wisconsin resident. Must submit statement of financial need and letter of recommendation from school official/instructor.

Eligibility Requirements: Full-time undergraduate accepted into the majors of Athletic Training (AT) or Clinical Laboratory Sciences (CLS), minimum 3.0 GPA and community volunteer service experience.
